All medication permission forms must be completed each school year.

Over The Counter (OTC)  Medications At School:

In compliance with Board Policy and State Law, OTC medications cannot be administered to students without signed consent from a parent/guardian. Consent must include name of the medication, dose, when to be given, and reason for use. OTC medications are defined as any product which contains an FDA drug label that can be purchased without a prescription. Examples include, ibuprofen, tylenol, and cough drops.

Prescription Medications At School:

In order for students to receive prescribed medications at school, the health office must first be provided with a signed order by a physician and signed consent from parent/guardian.
